This page contains a list of all provider offices, Express Clinics, and other Summit facilities that are closed or have temporarily changed hours due to holidays, weather, illness, staff meetings or other unforeseen circumstances.
Please check this page for important information on the day of your appointment by clicking the alert bar at the top of any page on the summit website.
If you have appointments at more than one office (for example, a provider office and diagnostic imaging center), please check for both locations. If you do not have a scheduled appointment and need to see a provider, visit our Express Clinics Appointments page to schedule an appointment with the clinic closest to you.
