please contact the physician “on call” for your regular doctor's
practice location first. He or she may advise you to go to one of the
Express Clinics. You may also call the Express Clinics directly for an
appointment if your regular primary care physician is unavailable.
At the completion of your evaluation, a summary of the findings and
recommendations of the Express Clinic physician (or mid-level
practitioner) along with any lab or X-ray reports will be sent to your
