Patients are seen by appointment only and by referral of your primary care physician. Please telephone 24 hours in advance if you will be unable to keep the appointment, since that time has been reserved for you. Payment for services rendered is due at the time of your visit, unless you are covered by an insurance plan that does not require a co-payment. Please come prepared to make your co-payment at each visit.
